Part Four

Two kinds of people are suffering in the medical care community in Las Vegas. These are those physicians who are ethical and moral and try their level best to provide quality and efficient medical care to their patients, despite the negative circumstances which surround them, and patients, who immoral, unethical greedy physician’s service. We, who live in Las Vegas lose and will lose a lot more in near future.
It is of utmost importance, the practices by greedy physicians and lazy nurses in Las Vegas be stopped. I request to the population of Las Vegas; go to local politicians, legislators etc and request the need to stop the above practices, as we, the citizens of USA and living in Las Vegas, must not be treated with such blatant underhanded greedy doctors, medical insurance companies, hospitals and lazy non physician staff.
We have a right to proper medical care and this right if not given to us must be taken. I request that when you go to clinics, hospitals or any other medical care facility, demand to see the physician appointed to you, demand to look at your charts, demand to speak with physicians who have billed you and if you do not recognize the physician’s name who has billed you or use name or signature is in your chart, confront the hospital, clinic authority and confront that physician. Part Three

Nurses and other medical staff in several hospitals in Las Vegas, prefer physicians who do quick consultations, as not much work is put in the way of nurses and other medical staff. These medical personnel on a regular basis make complaints on physicians who work ethically, morally and do what is right by the patient, since by doing what is right, work load of non physician medical staff is increased, as they now have to be involved in efficient patient care. This is not acceptable by non physician staff and such physicians must be commented on, ridiculed and non cooperation banner must be hung.
So what can a caring physician do? Because these physicians want to provide good and excellent patient care, they are harassed at several levels. The physician ultimately complies or leaves Las Vegas or pushes on and takes the stress of harassment. Not many physicians take too such constant harassment without affecting their personal life. They lose their physical and emotional health. Sad and unfortunate scenario. Look, what greed has done to medical practice in Las Vegas. The care giver and the care taker, both undermined.
Physicians who practice unethically and immorally are doing so because they are greedy. With medical insurance companies, hospitals and corrupt and inefficient monitoring behind them, the soil is fertile and sky is the limit.
There are many Dr. Greed’s in Las Vegas, continuing their greedy practices openly, boldly and blatantly, and getting away with it. Most of these Dr. Greed’s make above $500,000 to a million, annually. They do so by increasing the number of patients seen per day. To these physicians quantity of patients seen per day is more important, as it brings more money in their pockets, than see less number of patients and provide quality care. Some Dr. Greed’s openly boast of seeing 80 to 90 hospital patients or more per day.
Sadly, there is not one, not even one monitoring agency, in the state of Nevada, in Las Vegas or in hospitals, who have the slightest desire to circumvent such practices. Of course for reasons quoted above. You scratch my back, I scratch your back. Wonderful

Part Two

IN LAS VEGAS, PATIENT CARE IS OUT THE DOOR. Involved in this business of medical care are medical insurance companies, hospitals and physicians of certain ethnic origin. All three beneficiaries are guilty of looking look away each others injudicious practices, as they complement each others financial desires.
I am a foreign physician in Las Vegas. I see often, how employers of physicians push their employee doctors to see patients in the shortest possible time, by this I mean, 5 minutes or under. If you do not comply, you are called inefficient and slow. Not up to standards.
No physician however well trained, qualified or experienced can provide appropriate care to patients, however minor the patients complaints maybe, in 5 minutes or under.
Physicians start their practice after finishing their residency or fellowship. They come with enthusiasm and work ethics, learnt through out their upbringing and years of medical education and training. Physicians of moral value, of ethics, who want to do right by the patients, are outnumbered to the ratio of 1 in 25, by free for all approach physicians, by physicians who evaluate patients while standing at the door, by physicians who do 5 minute consultations, by physicians who write patient charts without seeing patients etc
In Las Vegas, employer’s of physicians systematically, over a period of time, break the will of ethical and morally obligated physicians, who under constant pressure and harassment from their employers break down and accept the will of the employers and follow through. Unfortunately, this is how it is and this is a factual scenario.
Now you might ask, why do physicians not refuse to comply? Many do refuse to comply to immoral, unethical, uncaring and dangerous practices but see this from the physician’s viewpoint. If a physician eventually does not comply to short consultations, they are labelled inefficient and slow. They are considered non profitable.

Part One

In Las Vegas medical practice is a thriving business. This situation is the same all over USA, but is limited by monitoring agencies check and balance, although this check and balance could be improved further.
Since monitoring agencies in Nevada are inefficient to the extent that the monitoring barely exist, it is free for all in Las Vegas. But then what more can Las Vegans expect, as Las Vegas is built on the back ground of mafia, who all of us know, were not the most law abiding people. That culture over a period of 30-40 years has filtered down to the society we live in, in Las Vegas.
This does not mean that all of us living in Las Vegas are corrupt, but knowing what we know now, it is true that health care monitoring agencies are rife with corruption and inefficiencies. This is on several levels, within the community of Las Vegas and within state organization. Their official’s have been documented as saying that they do not know of inefficient health care or abuse of physicians in Las Vegas, are covering their behinds.
In Las Vegas the monitoring agencies are beyond corrupt, demonstrated by the recent news in Las Vegas Sun about abuse of foreign doctors (J1 doctors, as they are referred too) which went unchecked despite the knowledge of Division of Health and Medical Board of Medical Examiners and the devastating news of hepatitis scare.

Patient in the underserved population got served by highly qualified doctors, who under the duress of abuse continued to perform and provide excellent service. Much of this goes to the strength of the culture these doctors come from and their commitment to medicine. I have all praise for the J1 doctors without whom we would not have efficient medical care of the underserved population

While, I understand that the underserved population needs are to be met, this must not be at the expense of placing a J1 doctor in an environment where there is potential for abuse.
I guess Dr. Carl Heard in his enthusiasm, and I quote, ‘access to care is what we are here to do’, prefers not to delay the application of a J1 employer, than to make sure that the employer clarifies his application

What politically savvy, Dr. Carl Heard is saying is, that it is best to have a doctor in a potentially abusive situation, than not to have a doctor at all. I guess Dr. Carl Heard does not have son or daughter or relative, who in the past or at present is doing a J1 waiver or is a candidate for J1 waiver. I can tell you, if this was so, Dr. Carl Heard, would not have had the philosophy of providing medical care at expense of J1 doctor.

Instead of investigating the concerns of your fellow committee members further, Dr. Carl Heard like all dictators has taken the matter into his hands and decided against it, hiding behind the pretext of providing care.

I wonder who gave the authority to Dr. Carl Heard to overrule concerns of Matheis and Ford. I guess Dr. Carl Heard is doing what is commonly known in bureaucracy as, playing a leadership role.

Dr. Carl Heard, for god’s sake, it is high time you do what is right, instead you beat around the bush.

Dr. Carl Heard, do your job. Look for written verification of concerns which Larry Matheis and Caroline Ford have about the application. Should you decide to do so, make sure you verify judiciously, as any abuse, however minor, will go on your head and when this happens, I would not like to see you, like other concerned bureaucrats, as in the Division of Health, hide behind this or that regulation. Your’s is a responsible job, do it responsibly.

And stop listening to lobbyists, as you had in the past. Lobbyists as you and I would agree, are paid, immoral and unethical humans, who are there to influence you and the likes, by hook or by crook, so that they can succeed in their ethical or unethical, moral or immoral agenda.

Caroline Ford and Larry Matheis: Are the two of you so weak and meek that Dr. Carl Heard can overrule your correct concerns. Bureaucracy at its best, I guess. If you are going to be weak and meek than you will be empty voices which Dr. Carl Heard will control, in which case you might as well not be in the committee.
