What has happened in this setting is incomprehensible. I am a registered nurse who works at an endoscopy center in another state. While it is unclear from the information whether this center used anesthesiologists to administer anesthesia or if it used registered nurses administering concious sedation, narcotic records are in place and the amount of narcotic given to any one patient from a single-dose vial is recorded along with the waste of any unused portion. I find it hard to believe that single-dose vials were used on multiple patients since 2004 and this was never discovered or reported.
