Former Republican U.S. presidential candidate Mitt Romney waits for the start of a welterweight bout between Manny Pacquiao and Juan Manuel Marquez at MGM Grand Garden Arena on Dec. 8, 2012, in Las Vegas.

SALT LAKE CITY — Utah officials say a clerk's mistake caused former Republican presidential nominee Mitt Romney to sign a form that would have registered him as an unaffiliated voter living at an outdated Utah address.

The Salt Lake Tribune reports the mistake last month involved a pre-printed registration form Romney signed while obtaining a driver's license.

The form had Romney's name pre-filled, along with the address of a home the former Massachusetts governor sold five years ago. Other spaces are blank, including the choice of political party and a question that asks if Romney is a U.S. citizen.

State elections director Mark Thomas says the error was caught before the registration was processed.

Romney is being sent a new form, which an aide says he will fill out as a Republican.

Romney has a new home under construction in the Salt Lake City suburb of Holladay.
