Tom Scott, 30, was killed in Tuesday's accident at the Newmont Mining Corp.'s East Carlin mine north of Carlin, company spokesman Doug Hock said.
Scott was driving a water truck Monday when he backed into a drift area in the main tunnel and fell down the shaft.
He was ejected during the fall and died from massive head injuries.
The U.S. Mine Safety and Health administration is continuing to investigate, Hock said Wednesday.
It marks the first mining death in Nevada in the new year. Nevada's mining industry ended the year 2000 with six fatalities, down from nine the year before.
